Description

When additional information is required from a claimant, the Additional Information letter can be used to specify what information is required. One, or more, of the following items can be included by the user.


Content

  • Did you receive the merchandise or service?
  • What was the date you contacted the merchant to cancel the account?
  • What is the cancellation confirmation number?
  • Have you requested a refund? What was the merchant's response?
  • Please include any additional supporting documentation including email communications.
  • Please include a copy of your receipt or any documentation stating the terms of your agreement.
  • Please provide proof of payment made by other means.
  • Please describe the item you purchased.
  • When was the item supposed to be delivered and how was it to be delivered?
  • What was the date you contacted the merchant?
  • Have you received any merchandise other than the sample/trial? If so, please contact the merchant to obtain a return merchandise authorization number in order to return the merchandise via trackable means.
  • We are unable to assist you without specific information; it is essential that you contact us promptly at the telephone number provided.
  • Please send an explanation as to how the charge you authorized differs from the charge billed by the merchant.
  • If you have not yet received the merchandise, please contact the merchant to obtain the expected delivery date and to inquire as to whether they will be issuing credit for any expedited shipping costs billed to you. If you intend to reject delivery due to the delay, please review the merchant's return policy.
