Written Statement of Unauthorized Remotely Created Check

Description

The Written Statement of Unauthorized Remotely Create Check form is required to support fraud Check claims. This is used by the Paying Bank to request a refund when they believe a remotely created check was unauthorized, the amount is incorrect, or the signature is missing or forged. An adjustment request must be made to the Federal Reserve if an URCC is found

The form is included with the confirmation letter as an attachment/enclosure. The statement section of the form will be pre-selected, based on the answers provided during intake.
